Good Faith Estimate & No Surprises Act Notice

At Hummingbird Therapy, I am committed to transparency around the cost of services and compliance with the No Surprises Act.

Your Right to a Good Faith Estimate

Under the federal No Surprises Act, clients who are uninsured or who choose to self-pay have the right to receive a Good Faith Estimate (GFE) for the expected cost of services.

This estimate will outline the anticipated cost of therapy services based on the frequency and type of sessions discussed at the beginning of treatment.

You will receive this estimate in writing before services begin or upon request.

What This Means for You

You have the right to:

  • Receive a clear estimate of expected costs before services begin

  • Ask questions about fees at any time

  • Request a Good Faith Estimate before scheduling or continuing services

While therapy is often an ongoing and collaborative process, your financial expectations will always be discussed openly and without pressure.

Important Notes

  • The Good Faith Estimate is not a contract for services.

  • Actual costs may vary depending on frequency, duration, and clinical need.

  • You will never be charged significantly more than the estimate without prior discussion and agreement.

If You Receive a Bill That Is at Least $400 More Than Your Estimate

You have the right to dispute the bill under the No Surprises Act dispute resolution process.

More information about this process is available at: https://www.cms.gov/nosurprises

Questions

If you have any questions about fees, estimates, or billing, please reach out anytime before or during the course of therapy.
